Made my own - PowerPoint (or OpenOffice), print on card stock and cut down to size.
Only one-sided, however, but I usually use the back for a personal note about my equipment, the contact, or whatever.

Subject: [amsat-bb] Satellite-specific QSLs
Anyone have a good source for small run (100-250), photo QSL's with a satellite-specific report format on them? Or does everyone just custom design their own (and use a full-custom supplier (front and back))?
