I scrolled to the bottom of your message and used the link provided by JoAnn and filled out the survey. The buttons on the survey pages have French labels on them. Use your intuition and should have no problem determining what they are for.

Since the survey is largely in French, what do you expect? Although

questions are in poorly worded English, everything else in in French.
Most
of the survey requires answers where there is no "None of the above" or equivalent. I gave up trying...
