I'm afraid I haven't, but I'm really impressed with the one-off cost of www.batchpcb.com "$10 setup fee (per order - appears as a handling fee), 2 layer - $2.50/sq. inch 4 layer - $8.00/sq. inch". If you could get some Eagle files from someone, perhaps this service might help.
(If anyone has experience with this or other similar shops, I'd love to hear about it.)

Check out FAR Circuits, as well... I can't remember what I paid to have them make the FOD-Track boards several years ago (which are now in their regular lineup...), but I just had to send a negative of the board & a reasonable (I thought) fee, and the deed was done.

Hi Ronald The artwork in the article is from the Maxim application note and its larger than required Hi ! Hence I made my own layout using express pcb. If you observe my layout is slightly different and I have added the 3.3 v regulator (78m33) on the pcb. For sharpness I had to print at 2x or more on the laser printer and then reduce it manually while making a transpererncy film for silk screen for photo etch method as I made a few pcbs for trial and tesing. It indeed is good and the pcb layout is sharp. Best 73 Pop VU2POP
